Overview

Health professionals who have been erased for more that 2 yrs upwards to 4 yrs may be restored under the following conditions:

[This process would require temporary registration equally a student with the HPCSA] One remediation opportunity for each assessment is permissible (i.e. 1 set of OSCEs and one adult and/or paediatric simulation). A practitioner whose proper name has been erased from the register and had not been practising for a period of more than two years volition be required to work under supervision in an HPCSA accredited laboratory for a period equivalent tosix months (1000 hours) the following procedure would apply: Complete Restoration Class 18. Payment of restoration fees and any other fees outstanding. The applicant be restored in terms of supervised practice immediately on submission of the awarding grade (Class 18, Grade 18A, Form 18C, Form 18D) and payment of penalty fees; They accept to piece of work under supervision of a registered Medical Technologist at an accredited laboratory, for a menstruum of at to the lowest degree vi (half-dozen) months; The supervisor should on completion of the period of 6 months, submit a progress study with a recommendation as to the lifting of restrictions or a further period of supervised practice; and The progress written report will exist considered by the Professional Board or Education Committee During the period of supervised practice the bidder will non be allowed to be self-employed or practising in his or her ain individual practise. Upon receipt of a positive supervisory study or portfolio by the supervisor to the satisfaction of the Education Subcommittee, his or her name volition exist restored to the category of registration that applied prior to the erasure of his or her name from the register.

SUMMARY OF PROCESS The supervisee will be required to identify and request a suitable supervisor to oversee him or her during the period of supervised practice. Restoration after Removal or Interruption of name from the register for a period of more than three years – (Non-practising)If a practitioner'southward name was removed from the register or his or her registration suspended for a menses of more than iii years and he or she was not practising the post-obit procedure would employ: Completion of the required awarding forms: Form 18 and Class 18 A OCP Payment of restoration fees The practitioner would be restored in the category  supervised practice for a period of at least six months (Equivalent to at least  grand hours) Apply to the Board as per the guidelines in Course 18 B OCP for approval of the supervising practitioner by completing and submitting Form 18 C OCP Once the Educational activity subcommittee had granted approval  of the supervising practitioner the period of supervised do could  commence Submission of a report by the supervisor upon  completion of menses of supervised practice – Grade 18 D OCP Supervisory Report If the standard of the written report is not accepted by the  Board a portfolio should exist compiled as per Form 18 Due east OCP Portfolio Guidelines Submission of the portfolio by the supervising  practitioner as per Form xviii F OCP portfolio Submission Form. Upon receipt of a positive supervisory report or portfolio by the supervisor to the satisfaction of the Education Subcommittee, his or her name will be restored to the category of registration that applied prior to the erasure of his or her name from the register.

D. Restoration of Names of Assistants and Technicians:Separate guidelines employ in the case of the restoration of names of administration and technicians in view of the fact that they are just entitled to practise nether supervision.

E. Erroneous registration, fraud, disciplinary action or harmProcedures in these guidelines do not include information relating to erasure due to erroneous registration, registration through fraud, disciplinary activity or damage.

Optometry & Dispensing Opticians

Practitioners who were off the register and had been out of clinical work for longer than v years demand to pass the Board exam earlier they may be restored to the register.

Practitioners who were off the annals and had been practicing outside the country in the past five (5) years or more than need to provide similar requirements (a) –(g) in order to exist restored to the register and

D. The supervision period entails the post-obit: A registered practitioner in the relevant profession will be required to supervise the applicant during this period and study to the Board regarding his or her competence upon completion of the menstruation of supervised do past submitting a portfolio in respect of his or her activities during the menstruation of supervised practice. The written report should include an indication whether a further menstruum of supervision is required. Voluntary Erasure

Any wellness professional may apply for voluntary erasure from the annals in writing earlier 31 March. If awarding is made on or after 1 Apr of that yr the annual fee for that twelvemonth must outset be paid and then voluntary erasure be requested with event from the post-obit twelvemonth.
